Registering for the Conference
ANZPA Conference 2009 Registration Brochure
You can register in the following ways:
2. PRINT and fill out the ANZPA Conference 2009 Registration Form, (PDF, 64KB). Then either:
* MAIL to ANZPA Conference Registrar, 24 Frame Street, Northeast Valley, Dunedin 9010. New Zealand. Or
* FAX the completed form to: +64 3 473 9365
3. Download the ANZPA Conference 2009 TEXT Registration Form (WORD, 80KB), open the file, fill in the information required, save the file. Then either:
* EMAIL to confregistrar@anzpa.org as an attachment. Or
* MAIL to ANZPA Conference Registrar, 24 Frame Street, Northeast Valley, Dunedin 9010. New Zealand. Or
* FAX the completed form to: +64 3 473 9365
How to pay
You can pay in one of the following ways:
From New Zealand
• Mail a New Zealand cheque made out to “ANZPA Conference 2009”
• Make a direct deposit into our Westpac Bank Account: 03 1726 0017121 001 (please use your surname and initial as a reference, and email us to let us know the date you have paid) – please note the 001 suffix
• Fax or mail us your credit card details including card number, card name and expiry date and the amount you are paying
From other countries
• Mail us an International bank draft made out to “ANZPA Conference 2009”
• Fax or mail us your credit card details including card number, card name and expiry date and the amount you are paying
• Make an International inter-bank transfer into our account:

Refunds
A cancellation fee applies. Pre and Post Conference workshop fees are non-refundable after 21 December 2008.
